Is It Sciatica? How to Tell What's Really Going on With Your Back and Leg Pain

Most people describe it as a sharp, burning, or electric sensation that starts in the lower back or buttock and runs down one leg, sometimes all the way to the foot. You might also notice tingling, numbness, or weakness along that same path. It typically affects one side of the body, and sitting for long periods often makes it worse.

A Quick Self-Assessment

Ask yourself these questions. Do you have pain that radiates below the knee on one side? Does sitting, sneezing, or coughing intensify it? Is there any numbness or tingling in your leg or foot? Do you feel weakness when trying to flex your foot? If you answered yes to two or more of these, sciatica may be worth exploring with a specialist.

Conditions Often Mistaken for Sciatica

Not all leg pain is sciatica. Hip bursitis causes pain on the outer hip that can travel down the thigh. SI joint dysfunction creates deep, aching pain in the lower back and buttock that mimics sciatic discomfort closely. Piriformis syndrome, sometimes called pseudo-sciatica, occurs when the piriformis muscle compresses the sciatic nerve rather than a spinal disc. Getting the right diagnosis matters because each of these conditions responds to different treatment approaches.

True Sciatica vs. Pseudo-Sciatica

True sciatica, or lumbar radiculopathy, originates from nerve compression in the spine, often from a herniated disc or bone spur. Pseudo-sciatica produces nearly identical symptoms but the compression happens outside the spine, in the soft tissue. Both are very real and both respond well to chiropractic care when properly identified.
